When you click on the "Download Printable PDF" link above, you will be able to fill out the form on your own computer. You can then email it directly to FUDTA, using a button on the form; or you can print it out on your own printer, again using a button on the form; or you can fill out the form partially, save your work, and then reload it to finish at a later time.

Note that this membership form is controlled by the CTA and NEA, and FUDTA has no control over its appearance. You must use the same one-letter-per-box method of filling out this form on the computer as you would if you were filling it out with pen and ink.
